The format
What is a SOAP note?
SOAP is the most widely used progress note format in outpatient therapy. Each letter stands for one section of the note.
Subjective
What the client reports — their mood, presenting concerns, experiences since the last session, and any relevant personal history they share.
Objective
What you observe — affect, appearance, behaviour, and any formal assessment data or mental status observations from the session.
Assessment
Your clinical interpretation — how the client is progressing toward treatment goals and your professional judgment about the presenting concerns.
Plan
Next steps — interventions used this session, homework assigned, referrals made, and what you intend to address in the next session.
Example output
A real SOAP note — generated in 4 seconds
From 8 bullet points. No templates. No boilerplate. Only what you entered.
Subjective
J.D. reported increased anxiety this week attributed to workload and upcoming deadlines. Client described constant worry and difficulty relaxing, and reported sleeping approximately 4–5 hours per night. Client denied suicidal ideation and self-harm.
Objective
Client appeared restless and fidgety throughout the session. Speech was rapid at times. Affect was anxious but appropriate to content.
Assessment
J.D.'s reported anxiety and sleep difficulties appear connected to current occupational stressors. Difficulty relaxing and reduced sleep are likely reinforcing the cycle of worry. Progress toward treatment goal of reducing generalised anxiety is ongoing.
Plan
Practised grounding exercise (5-4-3-2-1) in session. Discussed identifying and challenging unhelpful automatic thoughts using CBT thought records. Assigned thought log as homework. Continue weekly sessions.

How it works
From session observations to signed SOAP note
Write your observations
After the session, add 4–8 bullet points — what the client said, what you observed, what you did. No full sentences required.
Notelyx generates the SOAP note
The AI structures your observations into a complete SOAP note, placing each detail in the correct section. No hallucinations — only what you entered.
Edit, sign, and export
Review the draft, make any edits, then sign to lock it. Export a clean PDF for your EHR or client file in one click.
Why Notelyx
Not just another AI note tool
No audio recording
Notelyx never records or transcribes your sessions. You write the bullets, the AI structures them. Nothing your client didn't already consent to in your standard disclosure.
No hallucinations
The AI is strictly instructed to use only what you enter. It will never add diagnoses, infer risk factors, or introduce clinical language not present in your observations.
HIPAA compliant with BAA
All data is encrypted at rest and in transit. Raw observations are automatically deleted 24 hours after generation. Every user receives a signed BAA.
Works alongside any EHR
Export a clean PDF and upload it to SimplePractice, TherapyNotes, Jane, or any other platform. No direct integration required.
FAQ
SOAP note questions
What is a SOAP note in therapy?
A SOAP note is a structured clinical documentation format with four sections: Subjective (what the client reports), Objective (what the clinician observes), Assessment (clinical interpretation), and Plan (next steps and interventions). It is the most widely used progress note format in outpatient therapy.
How long should a therapy SOAP note be?
A typical outpatient therapy SOAP note is 150–300 words across the four sections. The Subjective and Assessment sections tend to be the longest, while Objective is usually brief (one to two sentences) and Plan lists concrete next steps.
Are AI-generated SOAP notes HIPAA compliant?
HIPAA compliance depends on the specific tool. Notelyx is built to meet HIPAA requirements: all data is encrypted at rest and in transit, raw observations are deleted 24 hours after note generation, the AI never trains on your data, and every user receives a signed Business Associate Agreement (BAA).
Can I edit a SOAP note after it is generated?
Yes. Notelyx generates a draft that you can edit section by section before signing. Once signed and locked, it becomes a permanent record. You can also regenerate the entire note from your original observations.
What information do I need to write a SOAP note?
You need 4–8 bullet points covering what the client reported, what you observed, any interventions you used, and what you plan for next session. Short observations are enough — no full sentences required.
